Patient Perspectives on Social Needs Screening and Navigation Services in Serious Illness Care

A national survey of 2,510 patients with chronic and complex conditions found that most faced significant financial and social challenges, such as difficulty paying for medications, housing, or utilities. Despite these needs, only two-thirds discussed them with providers, and just one-third felt comfortable being asked. Patients valued navigation services—particularly for financial assistance—with nearly all reporting them as helpful, though cost and awareness limited uptake. Key priorities included support accessing care, connecting to resources, and fostering trust and communication. Findings highlight the need to embed affordable, patient-centered navigation into care delivery to improve outcomes and reduce inequities.
